D2.456 - Chronic sinusitis with nasal polyposis: Add-on aspirin desensitization useful in dupilumab therapy?
Background
Nasal polyposis (NP) is a an uncomfortable condition in chronic sinusitis with relapsing inflammation despite of sinus surgery, intake of steroids and aspirin desensitization. Monoclonal antibodies (MAB) offer a new and prosperous treatment option in NP. Up to now there are no data of a combined aspirin and MAB regimen.
Method
We examined 61 patients not responding adequately to aspirin desensitization then on dupilumab therapy. One group stuck on aspirin as add-on, the other received dupilumab solely. Quality of lifeparameters, rhinoscopy and NO levels were surveyed.
Results
SNOT-scores, polyp and nasal inflammmation scores, FeNO and nasal NO levels improved significantly, but showed no difference before and after treatment of at least one year between both groups.
Conclusion
Dupilumab causes a significant improvement in the majority of chronic sinusitis patients with NP. Additional aspirin reveals no change in outcome under therapy. As it may be seen as a causal therapy, the further course without dupilumab should be observed.
