001699 - Occupational contact dermatitis to non latex gloves

Case report

A 34-year-old woman, surgeon, reports that suffer from microvesicular lesions in the interdigital area, as well as on the fingers, back of the hands and wrists for 6 months, after 6-8 hours of using various gloves in the operating room, which improve over several days with application of topical treatment with fusidic acid/betamethasone every 12 hours for periods of 2 weeks and without the use of gloves.

Now, with adaptation in her job, in which he undergoes consultation but has not been active in the operating room for 3 weeks.

The patient reports a history of atopic dermatitis in childhood that has been controlled for years and has not flared up

The patient was refer from occupational medicine for adaptation of her job and the type of gloves she can use.

Method and Results

Patch test with TRUE TEST was performed with negative results at 48, 72 and 96 hours. Patch tests with different sterile gloves available in hospital operating rooms were also perfomed:A. Latex-free gloves: 1.Profeel DHD (polychloroprene), 2.Biogel Skinsense (polychloroprene), 3.Sensicare PI (polyisoprene), 4.Gammex non latex (sensoprene), 5.Gammex non latex green dermashield (polyisopreno), 6.Naturflex neo 2.0 (neoprene without powder or accelerators), 7 Epic N surgical (accelerator-free nitrile) and B. gloves with latex: Protexis latex without powder. The result was positive (+) for 4.Gammex non latex (sensoprene), 5.Gammex non latex green dermashield (polyisopreno) and Protexis

Latex at 48 hours and Sensicare Pi (polyisoprene) at 96 hours. Being negative for Biogel Skinsense and Profel (polychloroprene).

A challenge test was perfomed with Naturflex Neo 2.0 (polyvinyl chloride) with positive result

The patient has used Biogel Skinsense gloves and Profeel with good tolerance and avoid latex gloves because the atopic condition and acute lesions.

Conclusions

Nowadays, the available literature on allergy to latex-free gloves is scarce and the technical sheets of the gloves offer little information about their composition.

The patient tolerate polychloroprene gloves and can use it in the operating room again.
