100057 - Specific IgE molecular sensitization pattern of House dust mite allergen-components by Component Resolved Diagnosis [CRD] in 318 patients of allergic diseases [Respiratory allergy [168], Food allergy [80], Atopic dermatitis [70]
Background
40 groups of HDM allergens are identified and have been registered in WHO / IUIS nomenclature sub-committee. Our study is done on preliminary prevalence of allergenic profile of HDM allergens [major HDM components (Der p 1, Der p 2, Der p 23, Der f 1, Der f 2), mid-tier allergens (Der 5, Der p 7, Der p 21), tropomyosin (Der p 10, Per a 7, Pen m 1, Ani s 3, Blo t 1), arginine kinase (Pen m 2, Bla g 9, Der p 20)]
Method
- We analyzed serum using ALEX2 microarray platform for determination of disease-eliciting molecules and potential cross-reactivity between allergens. The study focused on HDM allergen sensitization profile in northern India for accurate diagnosis and HDM allergen immunotherapy.
Results
- Among 318 patients, 35% had respiratory allergy, 25% food allergy with s/s of UAA and 22% with atopic dermatitis / eczema.
Group A major allergens- Der p 1, Der p 2, Der p 23, Der f 1, Der f 2
Group B mid-tier allergens-Der 5, Der p 7, Der p 21,
Group C cross-reactive allergens- tropomyosin (Der p 10, Per a 7, Pen m 1, Ani s 3, Blo t 1), arginine kinase (Pen m 2, Bla g 9, Der p 20)
The molecular pattern found- was only Group A was pos in 11.9%, A+C- 14%, A+C=3.1%, A+B+C= 5.6%. Group A was 78.2% positive i.e., who need HDM AIT while 21.8% were from Group B&C where AIT is not indicated.
Conclusion
HDM AIT is a complex journey. HDM components are sufficient to diagnose more than 90% of allergic patients. Sensitization to HDM allergen was most common with major determinant component Der p 1, Der p 2, Der p 23, Der f 1, Der f 2. Increasing titers of these specific IgE components was also correlated with the risk of respiratory allergy [allergic rhinitis and allergic asthma] &for precise allergen immunotherapy.
